Submission Requirements
In order to expedite your quotation requests, we need the following information at a minimum:
- Completed New Business Supplemental Application
- Narrative description of operations
- Completed ACORD applications
- Currently valued loss runs for the past 5 years for each coverage to be quoted, along with a narrative of any claim exceeding $25,000
- Corresponding past 5 years of exposure data
- Applicable Supplemental Applications
- Current Workers’ Compensation Experience Modification Rating Worksheets (if applicable)
